We are so happy that you have chosen Our Lady of Mercy Academy!
In order to complete your application, we require the following steps to be taken for new families joining us:
Students entering Grades 1 - 7 must present their most recent Report Card and any relevant educational records (such as IEPs, 504s, etc.)*
Parents of students entering Grades 1 - 7 must make an appointment to meet with the Principal or take a tour.
Students entering Grades 1 - 7 are asked to spend a day with us and take a placement exam.
Birth Certificate
For entry to Pre-Kindergarten 3, children must be three years old on or before Oct. 1st
For entry to Pre-Kindergarten 4, children must be four years old on or before Oct. 1st
For entry to Kindergarten child must be five years old on or before October 1st
Baptismal Certificate if applicable
Current Physical Exam Form - Please send in most current physical exam papers before the school year begins.
Current Immunization Record - Please know religious exemption is not accepted in Catholic Schools under the Archdiocese of Newark. To be eligible for enrollment, children must be up-to-date on vaccinations or have a set schedule of vaccines signed by your child's pediatrician.
In addition to the above items, we ask that the following forms are completed and returned along with your paid registration upon acceptance:
Transportation Forms - Bus service is provided through the Board of Education from each town. All families are required to fill out the attached form and return it to the school office. THE SCHOOL MUST SEND FORMS TO INDIVIDUAL TOWNS BY JANUARY OF THAT YEAR IN ORDER TO RECEIVE SUBSIDIES.
*8th grade transfer requests are handled on an individual case basis and must meet specific criteria.
